Refund Policy
By November 16, 2009 we will refund you your registration fee. We have a no refund policy after November 16, 2009.
Waiver
I, the undersigned parents or guardian, on behalf of the minor child listed above do hereby release and waive any rights, cause of action for damages of any nature whatsoever that may have or may arise against the Thanksgiving Basketball Camp, its staff, officers, directors, coaches, referees, and others appointed by or acting for such camp as a result of any accidents or injury involving my minor child while participating. If my child has any health problems, I have enclosed a health release form from his/her doctor. Further, I hereby authorize the Thanksgiving Basketball Camp and its representatives to seek and authorize medical care for my child.
